A $195,000 Quest class action settlement will resolve claims that a 2016 data breach compromised the information of thousands of patients.
The settlement benefits individuals whose information was obtained by an unauthorized third party in the 2016 Quest data breach.
The Quest data breach reportedly took place in November 2016 when unauthorized users gained access to Quest Diagnostics’ MyQuest application.
As a result of the data breach, the information of around 34,000 patients was allegedly compromised. Exposed data reportedly included names, dates of birth, telephone numbers, and even lab results including HIV results.
Plaintiffs in the Quest data breach class action claimed that the company failed to protect consumer information from being compromised. Additionally, the patients claimed that Quest did not provide “timely, accurate and adequate notice” of the data breach.
“Despite the fact that it was storing sensitive Private Information that it knew or should have known was valuable to and vulnerable to cyber attackers, Quest and its fellow Defendants failed to take adequate measures that could have protected user’s information,” the Quest class action lawsuit alleged.
Quest Diagnostics has not admitted any wrongdoing but has agreed to fund $195,000 in settlement payments to affected consumers.
The settlement provides up to $325 in compensation for individuals affected by the Quest data breach. Up to $250 can be collected per patient in compensation for actual monetary losses that resulted from the breach. An additional payment of $75 can be collected by patients who had their HIV test results compromised, regardless if actual monetary losses occurred.
In order to benefit from the settlement, Class Members must file a valid claim form by May 22, 2020. The deadline for exclusion and objection was Jan. 8, 2020. Final approval was granted to the settlement at the fairness hearing on Feb. 25, 2020.

Add Me please . I had a lot of lab work done
add me
They do what they want to.
Add me please
THE PHONE NUMBER THAT WAS POSTED TO CHECK IF YOUR ARE ELIGIBLE TO CLAIM IS BOGUS!!! ITS A CREDIT MONITORING COMPANY!!!! DO NOT CALL IT….I want to be involved with this lawsuit because I have had a lot of issues with Quest for years
To see if you are part of this lawsuit call this number :
1-800-491-5304
They will ask for your name and zip code. Good luck and quit posting “Add Me”. That does nothing.
add me
Add me in , I’m a long time quest user !!!!
I am long time quest user, how do we know if we qualify?
I HAVE BEEN USING QUEST FOR YEARS. I HAVE NOT BEEN NOTIFIED OF THE DATA BREACH. HOW WOULD I KNOW IF I HAVE BEEN AFFECTED.
Call 1-800-491-5304, and give her your name and zip code. She will look your name up in the database to see if you were sent out a letter that your information was breached. If it was you should have been sent out a letter which tells you how to submit a claim with the reference number on file. You cannot submit a claim unless you were assigned a reference number.
You would have received a letter with a reference number to file file a claim. You cannot file a claim without this letter. Call 1-800-491-5304 and give her your name and zip code and she will let you know if a letter was sent to you.
This is a credit monitoring service!
Did you receive a letter? Call 1-800-491-5403 and give her your name and zip code and she will check the database to see if a letter was sent.
How do we get our claim number if we did not receive one? I have been using quest for 15yrs!
If you didn’t receive one your information wasn’t breached. You can call 1-800-491-5304 and give her your name and zip code she will check the database to see if a letter was sent to you. If it was it has a reference number on it telling you to use that reference number on your claim that you can file online. You can only file a claim if you were sent this letter letting you know that your information was breached.
Who ever Lowary is is giving INCORRECT INFO…this is a number for a credit monitoring service
Please add me. I have used Quest for years and can provide proof of this
