UPDATE: September 2020, the Court decided to extend the supplemental claim period by 12 weeks in order to cover debonds that were fixed through Sept. 7, 2020. The new deadline for supplemental claims is Dec. 8, 2020. Class Members may begin filing for supplemental claims on Sept. 21, 2020.Â
Dentists have secured a $32.5 million settlement in a class action alleging 3M made defective dental crowns.
Class Members include dentists and dental practices in the United States who purchased 3M’s Lava Ultimate Restorative and used it as a dental crown before June 15, 2015. Dental patients must have also experienced debonding of the crown before June 15, 2020.
A group of dentists accused 3M of violating the terms of the warranty for their dental crown bonding product, Lava Ultimate Restorative. The product is a ceramic material that is meant to dry and form a crown in dental procedures.
Dentists complained that the 3M dental crown product was unbonding before its 10-year warranty.
As patients came back with issues, dentists who used Lava Ultimate Restorative allegedly had to repair and replace the dental crowns at their own costs.
3M fought back, asking a federal judge to dismiss the class action lawsuit arguing the claims were not specific enough.
Though the 3M dental crown class action lawsuit was trimmed of some claims, the company agreed to settle.
Under the terms of the class action settlement, Class Members can receive either full reimbursement for out-of-pocket costs related to the debonding of 3M dental crowns or $250 per debond.
To make a claim Class Members must submit a Claim Form online.
Class Members have until Aug. 8, 2019 to submit a claim for debonds that occurred on or before May 10, 2019. In addition, Class Members have until Sept. 15, 2020 to file a claim for debonds repaired between May 11, 2019 and June 15, 2020.
Class Members who wish to exclude themselves or object to the 3M dental crown settlement must do so by Aug. 14, 2019.
Who’s Eligible
Class Members include dentists and dental practices who bought and applied 3M’s Lava Ultimate Restorative before June 15, 2015. The product must have fully or partially debonded prior to June 15, 2020.
Potential Award
$250 per debond or 100% reimbursement for out-of-pocket expenses.
Proof of Purchase
Notice ID must be provided. Class Members claiming out-of-pocket costs must provide documentation of the expenses.

I have a total of 17 crowns in mouth of which 4 have came off. a bridge on one side that connects 4 teeth that is about to fall off any day. I dont have the money to repair this. The dentist i used has passed away since the work was done. Im sure i could or should be able to get some kind of information and reciept from his office maybe. Am I eligible for this.

I had a crown fall off and my dentist was out of town,
Applied by Dr Anton Richardt in 2016, then bottom right new crown came off 2017 on a Saturday, went to The Tooth Dr Naples, Fl and all he did was check other teeth for $164 and said to come back, its still off

I have had two crowns come off. One twice. All three times roughly 2 years of the original crowns. What do I do as a patient with this issue?
I have had 3 crowns that I have had to go back to the dentist and have fitted and bonded again. I had to pay for each one so add me to this lawsuit!
