Monte Carlo Restaurant FACTA Class Action Settlement

Monte Carlo Restaurant in Minneapolis, MN has reached a class action lawsuit settlement for violating the Fair and Accurate Credit Transactions Act (FACTA). As a result, anyone who used a credit or debit card at the restaurant between 2008 and 2010 may be able to receive a $30 gift voucher from the class action settlement.
The Monte Carlo Restaurant settlement will resolve a class action lawsuit, entitled Zaun v. Monte Carlo Restaurant, that alleges the restaurant violated FACTA by printing the expiration dates of its customers’ credit or debit cards on their receipts.
Under the Monte Carlo class action settlement, anyone who used a personal credit or debit card at the Monte Carlo Restaurant located at 219 3rd Avenue North in Minneapolis between November 23, 2008 and December 27, 2010, and received a copy of the receipt, can file a claim to receive a $30 voucher good toward food and beverages at the restaurant.
To receive a voucher from the proposed Monte Carlo FACTA settlement, you must submit a claim form in which you state, under penalty of perjury, that you used a personal credit or debit card at the restaurant and received a copy of the receipt during the Class Period. Claim forms must be postmarked no later than October 3, 2011.

Tried to enter claim July 5, 2011 and sight was under construction. Tried to enter claim today (October 1, 2011) and sight is closed. It seems like a very narrow window of opportunity to file a claim.
