Details: Download the claim form above. It’s quite straight forward. You’ll have to fill out your name and address. Once that’s done, try to dig up a Sports Authority receipt or credit card charge statement showing a credit card purchase from April 8, 2007 through April 30, 2008. If you don’t have one, that’s fine, you’ll just have to list on the form the date of purchase between April 8, 2007 through April 30, 2008, location of purchase (remember only purchases at California Sports Authority stores are included in this settlement,) type of credit card used (Visa, MasterCard, Discover, etc.,) and the last four digits of the credit card. Sign and date the form and drop it into the mail. That’s it! Now sit back and wait for your $20 voucher.
