
Continuum Health Alliance and Consensus Medical Group agreed to a class action lawsuit settlement to resolve claims they failed to prevent a 2023 data breach that compromised patient information.
The Continuum Health settlement benefits individuals who received a notice that their private information was compromised in the Continuum Health data breach between October 18 and 19, 2023.
The Continuum Health data breach allegedly compromised sensitive patient information such as Social Security numbers. Plaintiffs in the data breach class action lawsuit claim the companies could have prevented the breach through reasonable cybersecurity measures.
Continuum Health is a healthcare company that provides services to patients in their homes or in skilled nursing facilities. Consensus Medical Group is a physician group that partners with Continuum Health.
Continuum Health and Consensus Medical Group have not admitted any wrongdoing but agreed to pay an undisclosed sum to resolve these allegations. They also agreed to make business practice changes to better protect consumer information.
Under the terms of the Continuum Health settlement, class members can receive a cash payment and medical data monitoring.
Class members who experienced documented losses as a result of the data breach can receive up to $5,000 in compensation. These losses may include fraudulent charges, identity theft expenses, credit monitoring costs and more. Documentation such as receipts, bank statements and police reports will be required to support these claims.
Class members who did not experience documented losses can receive an alternate cash payment estimated to be $75.
All class members are eligible for two years of CyEx Medical Data Monitoring. This service includes one-bureau credit monitoring, dark web scanning, identity theft insurance of $1 million, fraud resolution assistance and more.
The deadline for exclusion and objection is Feb. 17, 2026.
The final approval hearing for the Continuum Health data breach settlement is scheduled for March 16, 2026.
To receive settlement benefits, class members must submit a valid claim form by March 2, 2026.
Who’s Eligible
Individuals who were sent a notice of the Continuum Health data breach indicating that their private information was impacted.
Potential Award
Up to $5,000 in documented losses or $75 cash payout.
Proof of Purchase
Documentation of losses, such as bank statements, credit card statements, receipts, etc.
