
Omni Family Health has agreed to a $6.5 million class action lawsuit settlement to resolve claims it failed to prevent a 2024 data breach that compromised patient and employee information.
The Omni Family Health settlement benefits individuals whose personal information was potentially compromised as a result of the data breach the company became aware of on Aug. 7, 2024.
The settlement also benefits a subclass of the same individuals who resided in California at any time between Aug. 7, 2024, and the claims deadline.
The Omni Family Health data breach class action lawsuit claims Omni Family Health failed to implement reasonable cybersecurity measures to protect patient and employee information from a data breach. As a result, hackers allegedly gained access to sensitive information, including Social Security numbers.
Omni Family Health is a nonprofit health care organization that provides services to communities in California’s Central Valley.
Omni Family Health has not admitted any wrongdoing but agreed to a $6.5 million settlement to resolve the data breach class action lawsuit.
Under the terms of the Omni Family Health settlement, class members can receive up to $5,000 for out-of-pocket losses related to the data breach. This includes unreimbursed fraud or identity theft losses, professional fees, credit expenses and other losses. Class members must provide documentation to support these claims.
Class members can also receive a pro rata cash fund payment. These payments will vary depending on the number of claims filed but are estimated to be around $105.56 per claimant at a 4% claims rate.
Class members are also eligible for two years of free medical shield complete services through CyEx. These services include comprehensive monitoring for the exposure of class members’ medical information, at least one bureau of credit monitoring services and $1 million in identity theft protection.
Members of the California subclass can claim an additional $100 cash payment in recognition of their claims under the California Confidentiality of Medical Information Act. These payments are subject to a pro rata decrease if there are not sufficient funds to make all payments for out-of-pocket losses and credit monitoring services.
The deadline for exclusion and objection is Dec. 5, 2025.
The final approval hearing for the Omni Family Health data breach settlement is scheduled for Feb. 26, 2026.
To receive settlement benefits, class members must submit a valid claim form by Jan. 5, 2026.
Who’s Eligible
All U.S. residents whose personal information was potentially compromised as a result of the data incident that the defendant became aware of on or about Aug. 7, 2024. Additionally, a California subclass has been defined as all settlement class members who resided in California at any time between Aug. 7, 2024, and the claims deadline.
Potential Award
Up to $5,000 for out-of-pocket losses, pro rata cash payment estimated at $105.56 per claimant and an estimated $100 cash award to California residents.
Proof of Purchase
Receipts or other documentation that support unreimbursed costs but excluding documentation that is “self-prepared” by the claimant. “Self-prepared” documents can be considered to add clarity or support to other submitted documentation.
