
C.R. Pharmacy Services, doing business as CarePro Health Services, has agreed to a $1.3 million class action lawsuit settlement to resolve claims that a 2023 data breach compromised sensitive patient information.
The settlement benefits individuals whose personal information was potentially compromised in the CarePro data breach that occurred on or around Nov. 16, 2023.
The CarePro data breach was discovered on Nov. 16, 2023, and affected the unencrypted personal information of patients, including names, contact information, dates of birth, state IDs, Social Security numbers, driver’s license numbers, financial account information and medical and health insurance information.
Plaintiffs in the class action lawsuit claim that CarePro failed to protect their information from the data breach, and that the company should have known that its cybersecurity protections were inadequate.
CarePro Health Services is a home health care agency that provides services such as skilled nursing, physical therapy, occupational therapy, speech therapy and home health aide services.
CarePro has not admitted any wrongdoing but agreed to a $1.3 million settlement to resolve the data breach class action lawsuit.
Under the terms of the CarePro Health Services settlement, class members can receive up to $5,000 for documented monetary losses related to the data breach.
These losses can include out-of-pocket credit monitoring costs, unreimbursed losses associated with actual fraud or identity theft, unreimbursed bank fees, long-distance phone charges, postage and gasoline for local travel.
Class members can also receive a pro rata cash payment, which is currently estimated to be $100. The amount may increase or decrease on a pro rata basis after the payment of settlement administration fees, attorneys’ fees, documented monetary losses and credit monitoring and identity restoration services.
The settlement also provides two years of free three-bureau credit monitoring, dark web monitoring, identity theft insurance coverage for up to $1 million and fully managed identity recovery services.
The deadline to opt out of or object to the settlement is Dec. 3, 2025.
A final approval hearing in the CarePro Health Services data breach settlement will take place Jan. 23, 2026.

Who’s Eligible
Individuals whose personal information was potentially compromised in the CarePro data breach that occurred in November 2023.
Potential Award
Up to $5,000 in documented losses, plus a pro rata cash payment and two years of credit monitoring and identity theft protection services.

Documentation of losses, such as bank statements, credit card statements, invoices or receipts.
