A class action lawsuit investigation has recently been initiated over reports that patients have suffered fromย a flesh eating disease after taking several kinds of type-2 diabetes medication, including Xigduo XR.
According to the U.S. Food and Drug Administration (FDA), reports of a flesh eating disease have been linked to SGLT2 inhibitors, a class ofย type- 2 diabetes medicationsย that includes:
- Invokana
- Invokamet/Invokamet XR
- Farxiga
- Xigduo XR
- Qtern
- Jardiance
- Glyxambi
- Synjardy/Synjardy XR
- Steglatro
- Segluromet
- Steglugan
Overview: Flesh Eating Disease and Xigduo XR
In August 2018, the FDA released an announcement warning that type-2 diabetes medications that are a part of a class of medications known as sodium-glucose contransporter- 2 (SGLT2) inhibitors are linked to a flesh eating disease affecting the genital area of individuals.
The flesh eating infection is known as Fournierโs gangrene, or necrotizing fasciitis of the perineum. While the condition is not very common, they report it is a very serious condition. To help raise warnings of the possibility for infection, the FDA is requiring drug manufacturers to add these warnings to all SGLT2 inhibitor drug labels.
The need for the addition of flesh eating disease warnings have come from 12 patient reports. The cases that these patients were diagnosed with Fournierโs gangrene after taking SGLT2 inhibitors sometime between March 2013 and May 2018.
In its warning, the FDA asked patients who are on SGLT2 medications to seek medical attention immediately if they begin to suffer from side effects such as redness, swelling, or tenderness in the genital/rectum area. Other symptoms may also include a feeling of uneasiness and fever. While there are also only 12 cases that have currently been reported to the FDA, the agency warns that there may have been several more unreported cases.
The 12 cases of Fournierโs gangrene included seven men and five women. One fatality was reported in those that were diagnosed with Fournierโs gangrene, and all of the patients were hospitalized. Each individual was also required to undergo surgery, with some having to undergo multiple surgeries.
Fournierโs gangrene is a bacterial infection that affects the perineum. It is a structure that sits below the skin and covers fat, blood vessels, muscles, and nerves in the body. To first cause infection, the bacteria enters the body through an opening in the skin, such as a cut. Once it enters, it spreads quickly and the underlying tissue that it affects is quickly destroyed.
Throughout the last 30 years, there have been only six cases of patients developing Fournierโs gangrene. While the flesh eating disease has always been a risk factor for patients with diabetes, these resulting cases have not been a norm.
Symptoms that may also determine development forย Fournierโs gangrene includes:
- Fever
- Pain and swelling in the genital area
- Unpleasant odor for affected skin area
- Anemia
- Dehydration
- Crackling sound from affected area
