What is the Tepezza® lawsuit about?
The Tepezza® lawsuit multidistrict litigation (MDL) centers around claims that Horizon Therapeutics failed to warn patients that its thyroid eye disease drug, Tepezza®, could cause permanent hearing loss and tinnitus. The first Tepezza® lawsuit was filed in 2022 by an Illinois man who alleged he suffered irreversible hearing damage after receiving Tepezza® infusions. Since then, many more patients have come forward with similar claims.
As of June 2025, 221 lawsuits were pending in the Tepezza® MDL. While the MDL remains relatively small, it has grown steadily over the past year and is now entering a critical phase.
Four bellwether trials are scheduled to begin in 2026, with the first set for April. These early test trials are intended to gauge how judges and juries may respond to evidence and testimony. Both sides are actively working through discovery and narrowing down case selections. These trials could influence future settlement negotiations.
How is Tepezza® linked to hearing loss?
Initial clinical trials used to approve Tepezza® reported hearing problems in about 10% of patients. However, later research suggests the risk may be significantly higher. A 2021 study presented at the Endocrine Society’s annual meeting found that 65% of patients treated with Tepezza® experienced some form of hearing issue.
Reported symptoms included:
- Hearing loss (23%)
- Tinnitus or ringing in the ears (27%)
- Sensation of ear plugging (12%)
- Autophony—an unusually loud perception of one’s own voice (29%)
The study also reported Tepezza® could cause various types of hearing loss. Some patients experienced sensorineural hearing loss resulting from damaged inner ear hair cells. Others had patulous eustachian tube, a disorder where the channel between the middle ear, nose and throat remains constantly open instead of only opening to regulate ear pressure.
In some patients, hearing loss proved to be permanent, with symptoms persisting even three months after discontinuing the drug.
A February 2022 post-marketing safety analysis by Horizon Therapeutics confirmed these risks, despite earlier trials indicating a lower rate of hearing-related side effects (9.5%).
As a result, the FDA approved a Tepezza® label change in July 2023. The updated warning now includes “hearing impairment including hearing loss, which in some cases may be permanent.”
