Details: Relatively easy settlement to submit your claim for if you bought a pair of glasses for your 12 year old or younger child between 1/1/05 and 7/20/08. Head over to the Settlement Administrator website and click on “Voucher Request.” You will be asked to enter in your control number from your notification form if you received a card in the mail, or you can check the box labeled “Click here if you did not receive a class notice by mail” and click Submit. Next you’ll be required to enter in your child’s birth date (remember they must have been 12 years old or younger when you made your purchase between 1/1/05 and 7/20/08.) Next, you’ll need to enter in the date of purchase and the store location. On the next screen just verify the information is correct and click submit! That’s it! It’s as easy as that.

TCA Staff Tidbit: How about this one? I like it. There are a couple of reasons. First, if less then 5% (35,000) of the affected class (700,000) submit their claim than they’ll randomly send out additional $15 vouchers to claimants until 5% is reached. There’s a limit of three $15 vouchers that can be used for a purchase, so instead of only getting $15 towards the purchase of a pair of glasses, you may end up with $45. Also, it appears that the settlement vouchers will have the claimants name on it, but, ANYONE can use it. The only caveat is that if your name isn’t on the voucher you can only use one at a time. You can’t combine three for one purchase. That’s STILL helpful. If you’re not going to use yours check with a local Lions Club. They’re always giving away glasses to those who need them, so they might be able to use your voucher(s). No? How about your local Boys and Girls club? I’ll bet they have a kid who could use the help. Make sure to submit your claim! Even if you can’t use the vouchers, give them to an organization who CAN use it.
