Details: Head over to the Settlement Administrator Website and download the claim form. It's set so you can fill it out on your computer and print it out, but can't save the changes. There's an email address you can send your claim to ( This e-mail address is being protected from spambots. You need JavaScript enabled to view it ), but, you'll still have to print our your claim form and sign it before scanning the completed form and emailing it to the Settlement Administrators. They don't allow you to save the filled out copy of your claim form because they want to make sure you print it out and sign it before emailing it to them. To fill out this claim form you'll need to include:

Your membership number (this would be included in the email they sent you when you were signed up) OR

Program Name (WebLoyalty.com, Shoppers Discounts and Rewards or another program for example) AND the email address you used to enroll in the program AND

The date you enrolled OR the credit card type used (Visa, MasterCard, American Express) and the last four digits of the card charged.

Make sure to check the box to CANCEL your subscription if you haven't already (unless you use it of course), sign the form and send it in! TCA Staff Tidbit: This lawsuit is an interesting one in that it FINALLY takes some of these large, in our opinion, information farmer websites to task for what they've done. Unfortunately the claim process isn't the easiest. We'd also like to see them go back further than two months, but, $20 is $20 right? So, if you can, make your claim in this class action lawsuit settlement. Don't let these worthless spam creating data farmers get another dime out of you for worthless information.
